Combined Solid Waste Landfill Waste Receipt Report

PLEASE NOTE: This form shall be used by all landfill types to determine Perpetual Care Fund deposits; solid waste surcharge payments; and waste receipt data as required by Part 115, Solid Waste Management, of the Natural Resources and Environmental Protection Act, 1994 PA 451, as amended.  Please save a copy of this form to an appropriate location on your computer to use for the entire fiscal year (October 1 through September 30).  For more information Ms. Christina Miller, Solid Waste Management, Waste and Hazardous Materials Division, DEQ at 517-373-4741.
