Successful Futures – Michigan State University Extension Service

Successful Futures is a comprehensive prevention program to assess needs and provide services to children ages 0-3 and their families through referrals to appropriate community agencies.

Services provided are screening of all newborns and their families for risk factors, in-home assessment, medical, social, mental health, substance abuse services, parenting education, nutrition education, developmental newsletters, breast feeding assistance, emergency supplies, and other identified needs.

Any family with a child 0-3 years old may receive Successful Future services, but intensive in-home services are directed at families with identified high risk factors.

Who created the Successful Futures Program?

A collaborative group of 18 community agencies involved in a year long planning process to develop a comprehensive prevention program for children 0-6 years old. Successful Futures addresses the needs of children 0-3 and their families through a 0-3 grant from the Children's Trust Fund.

Doesn't SF duplicate existing community services?

No. In fact, the whole purpose of SF is to fill gaps in existing services and make appropriate referrals to other existing service providers. This is only possible with the support of existing community agencies who helped design Successful Futures.

What area does Successful Futures serve?

The 0-3 grant that funds Successful Futures is for Bay County. However, Arenac County has also applied for 0-3 grant funding from the Children's Trust Fund and is currently working collaboratively with the existing Bay County Successful Futures program to offer similar services to their 0-3 population and their families with a goal of integrating the two programs into one for the next grant cycle.

Who administers the Successful Futures program?

MSU Extension Bay County provides staff to administer the program. Bay County provides financial support services. There are four SF staff, two are housed at the Bay County Health Department (Intake and Referral Coordinator and Maternal/Infant Nurse) and two are housed at MSU Extension Bay County (Building Strong Families Program Assistant and Clerical support staff.)


Family Nutrition Program - Michigan State University Extension Service

The Family Nutrition Program (FNP) is a free program for Bay County residents brought to you by MSU Extension in cooperation with the Family Independence Agency & USDA. Programs are held at MSU Extension - Bay County, or you can meet with instructors privately, in your home.

Any Bay County resident who is eligible for or receiving Nutritional Benefits from FIA may participate in the Family Nutrition Program.

Mid-Michigan Community Action Agency, Inc.

There are nearly 1000 Community Action Agencies throughout the United States, all sharing a common goal of enabling low-income families and individuals to become self-sufficient. How each one accomplishes this goal varies, depending upon local needs and available resources.

The Mid-Michigan Community Action Agency, Inc. (MMCAA) works towards this objective by providing low-income individuals with the resources, skill knowledge, and motivation they need to improve their economic condition and develop strong family, social, and community interactions.

Established in 1966, MMCAA is a private, non-profit, corporation. Its staff and volunteers work together to provide a variety of programs that benefit all residents of its six county service area.

The services provided by MMCAA are offered through county outreach offices and child and family centers. These offices also provide information and referrals to assist residents in accessing other resources available in the community.
