Obtaining a License and Fees

In order to obtain a license applicants must:

  1. Complete annually MDA Controlled Atmosphere Sealed Storage Application.
  2. Mail completed application along will a fee of $35.00 per room before November 15 (rooms must be sealed on or before November 15)
  3. Applicant seals rooms, completes Room Closure Notification Card and mails to Grand Rapids requesting inspection.
