Pursuant to the Michigan Public Health Code, P.A. 368 of 1978, as amended, it is required to publish the names of disciplined individuals.
To comply with this requirement, the Bureau of Health Professions frequently publishes a Disciplinary Action Report (see below). The Disciplinary Action Report (DAR) lists the disciplinary actions taken against health professionals who are licensed and regulated by the various health boards within the Bureau of Health Professions. The report also includes updated information regarding licensees who have appealed the Board's action to a higher court. The report includes the names of the health professional, their professional license number, the type of disciplinary action taken, the effective date of the action and the general nature of the complaint.
To obtain a copy of administrative documents pertaining to a specific health professional or to obtain copies of previous Disciplinary Action Reports, write to: Freedom of Information Coordinator, Department of Community Health, Bureau of Health Professions, P.O. Final administrative disciplinary action is subject to judicial review.
