I have been a practicing audiologist for many years. What do I have to do to get my license?

Agency: Community Health


You will be required to submit:

  1. An application and application fee of $270.00.
  2. A transcript confirming the completion of a master's degree or a doctoral degree in audiology from a regionally accredited college or university. (The transcript must be sent to us directly from the college or university. We cannot accept a copy or a document submitted by the applicant.)
  3. Verification of successful completion of a minimum of 9 months of supervised clinical experience in audiology.
  4. Passage of the PRAXIS examination in audiology.
  5. ASHA or ABA certification in lieu of verification of items 2, 3 and 4.
