End-Stage Renal Disease Facilities (ESRDs)

Michigan kidney dialysis centers (also known as End-Stage Renal Dialysis or ESRD facilities) provide services to individuals with chronic kidney failure by offering dialysis treatment or education on how to perform dialysis at home.

ESRD facility services include support from physicians,nurses, nutritionists, and social workers. This interdisciplinary team works with each dialysis patient with the goal of living as normally as possible and maximizing their quality of life.

Two types of dialysis treatment may be performed:
1) Hemodialysis involves passing an individual's blood through a machine with a special membrane to remove toxic substances. Hemodialysis may be scheduled for three (3) or four (4) times per week according to physician orders and degree of kidney disease;

2) Peritoneal dialysis involves filling the abdomen with a dialysate using the natural abdominal membranes to filter toxic substances from the body. Peritoneal dialysis may be performed several times a day according to physician orders and degree of kidney disease.

ESRD facilities are not licensed by the state, but may be Medicare certified. Medicare pays for services provided by ESRD facilities that voluntarily seek and are approved for certification by the Federal Department of Health and Human Services, Centers for Medicare and Medicaid Services (CMS). CMS contracts with the Department of Community Health, Bureau of Health Systems to evaluate compliance with the federal ESRD regulations by periodically conducting surveys of these facilities. No Certificate of Need is required.

Application Process
Prior to construction, contact the Health Facilities Engineering Section (HFES) of the Division of Health Facilities & Services at (517) 241-3408 regarding submission of architectural plans. Sites must be inspected and approved by HFES and local fire authorities prior to occupancy.

Applicants seeking provider-based status must submit the following documents to the address listed in the "Contact Information" below:
  - ESRD Application/Notification And Survey/Certification Report (CMS-3427)

  - A narrative that includes:1) Plan of Operation (days open, hours, shifts, etc.); 2) qualification and responsibilities of operating staff; and 3) how ancillary services will be provided.


  - Home Dialysis Questionnaire (if applicable).

  - For provider based status, submit written documentation demonstrating compliance with the five criteria found in the Code of Federal Regulations, 42CFR, Section 413.174(c).

In addition, a certification survey cannot be conducted without an approved Medicare Provider/Supplier Enrollment Application (CMS-855). Forms should then be submitted directly to the Fiscal Intermediary, United Government Services, LLC for approval.

Federal regulations require that all ESRD facilities participate in Renal Network of the Upper Midwest, Inc. (Renal Network 11) activities. Information regarding Renal Network 11 may be found at www.esrdnet11.org.

Fees
There is no fee for a certification survey.
