  Facts About Peripheral Arterial Disease (P.A.D.)  A power point developed by Health and Human Services review key points about P.A.D. including risk factors, methods for diagnosis, forms of treatment and preventive measures. This power point can be useful for health professionals or the public.

  These  stroke education tools   were designed for EMS providers. They are intended to provide a quick reference for pre-hospital stroke care. Using these tools will enhance the use of the Michigan Stroke protocol, the Cincinnati Pre-hospital Stroke Scale, and determine when the patient was last seen normal.  The card includes a reminder in bold print to document the time the possible stroke patient was last seen normal.  On the back is another reminder for families to go to the hospital to provide important medical history information or to provide a name and contact information of someone who can.  The Report to ED section reflects the expanded window for treatment that is available in some areas of the state

  Blood Pressure Measurement Quality Improvement CD:  A free self-instructional CD based professional education program addressing standardized measurement procedure, equipment issues, common errors, testing of knowledge and interpretation of Kortokoff sounds and home measurement by patients. Contact hours (1.1 beginning June 18, 2008) are available for the program.

  Healthy Lifestyle Prescription:
Advice and resources printed on a prescription format that focuses on smoking cessation, healthy eating and regular physical activity. Designed for healthcare providers to assist them to provide consistent advice to their patient population, 2005.

  Heart Failure Patient Record
This patient health record was developed in conjunction with MPRO to improve communication with the heart failure patient after discharge between the family, physician and home care. You can reprint it or use this information as a model for your own program.

  Patient Health Record  
A wallet-size tri-fold health record for individuals to record health information, periodic tests and health goals. This was designed around the MQIC preventive recommendations/guidelines, 2005.
