This page provides information regarding the license look up web site as well as other ways to obtain information you are seeking regarding a license or registration.
The Bureau of Health Professions License Lookup web site gives the following information on licensees/registrants:
- Current license status (active, inactive or pending)
- License Number
- License issue date and expiration date
- An indication as to whether an open formal complaint currently exists
- Disciplinary action information
Note: If the status of a license shows active but the expiration date has passed, health care professionals who are licensed or registered under Article 15 of the Public Health Code have a 60-day grace period in which to renew their license/registration. Their licenses/registrations are still considered active within that period.
In addition to this web site, operator assistance for license verification is available by calling the Verification Unit at (517) 241-9427.
Written documentation is available as follows:
- Verification - Cost is $5.00 and includes the name, address, ID number, current status, original issue date and expiration date of the licensee or registrant. Your written request should be sent to: MI Department of Community Health, Verification Unit, PO Box 30189, Lansing, MI 48909, along with a check or money order payable to: State of Michigan.
- Certification - Cost is $15.00 and is only provided to other state licensing/registration boards. In addition to the information included in the verification, the certification includes examination information, original issue date, and other information required by state licensing/registration boards. Your written request should be sent to: MI Department of Community Health, Bureau of Health Professions, Attn: Credentials Unit, PO Box 30670, Lansing, MI 48909, along with a check or money order payable to: State of Michigan.
