Single Business Tax (SBT)   Sales, Use, Witholding (SUW)
To ensure privacy and security the following information is required:
  • Federal Employer Identification Number (FEIN) or Treasury Registered (TR)
  • Tax Period Ending (month and year)
  • Gross Receipts
  • Organization Type

  • Tax Amount due for Sales, Use on Purchases, Use on Sales and Rentals, Withholding
